Your 8D was rejected: how to revise and resubmit without starting a second argument

The email arrives with a short line: "8D rejected, please revise." Sometimes there is a list of comments. Sometimes there is one sentence. Either way, the clock on the complaint is still running, and the instinct is to fix the obvious wording and send it back the same day.

That instinct is usually what produces the second rejection. This post is about the resubmission itself: how to read the rejection, how to plan the revision, and how to send it back so the reviewer can close it quickly. (Earlier posts in this series covered weak D4s, audit findings and symptom-only actions. Here we assume the report already went out and came back.)

Step 1: Read the rejection as a list of open questions

A rejection is rarely "your report is bad." It is usually "I cannot sign this because I cannot see X." Before touching the document, turn every reviewer comment into a question you must answer with evidence.

For example:

• "Root cause not verified" becomes: what test, trial or data shows this cause actually produces the defect, and that removing it stops it?
• "Escape not addressed" becomes: why did our checks not catch it, and what changes there?
• "Actions not effective" becomes: what data, over what period, shows the defect has not recurred?

If a comment is vague ("insufficient analysis"), do not guess. Ask. A short call or a two-line email - "To make sure the revision addresses your concern, is the gap mainly in occurrence cause, escape, or verification?" - costs ten minutes and can save a full cycle.

Step 2: Separate wording problems from substance problems

Go through the comments and sort them into two piles:

1. Wording / presentation: missing dates, unclear owners, no part numbers, team not listed, photos missing, D-steps out of order.
2. Substance: cause not proven, actions not linked to a cause, containment not shown to be complete, no effectiveness data.

Pile one is quick. Pile two is the actual work, and it may need new data from the shop floor. Be honest about which one you are in. Rewriting a D4 more confidently without new evidence is a wording fix to a substance problem, and reviewers notice.

Step 3: Decide whether you need to reopen the investigation

Sometimes the rejection is right: the team stopped at the first plausible cause. Signs that you need to go back to the floor rather than back to the document:

• You cannot reproduce or switch the defect on and off with the cause you named.
• The cause does not explain why only some parts, lots, shifts or lines are affected.
• The escape answer is "inspection missed it" with no reason why.
• Your actions were implemented before the cause was confirmed.

Reopening feels like admitting failure. In practice, a resubmission that says "we revisited D4 and found an additional contributing cause; here is the evidence" tends to be received better than a defended original.

Step 4: Keep the revision traceable

Reviewers who rejected a report will compare the new version against the old one. Make that easy:

• Increase the revision and date it. Keep the earlier version on file.
• Add a short change summary at the top or in a cover note: comment, what changed, where to find it.
• Mark changed sections (highlight, change bar, or a "Rev B" tag next to the heading).
• Do not silently delete earlier statements that turned out to be wrong. Correct them visibly - "Rev A stated X; further analysis showed Y."

Step 5: Check the whole chain, not just the commented step

A change in one D-step ripples. If D4 changed, then:

• Do the D5/D6 actions still address the new cause?
• Is the containment in D3 still adequate for the new understanding of scope (other part numbers, other lines, stock in transit)?
• Do the PFMEA and control plan updates in D7 reflect the revised cause?
• Does the effectiveness check in D6/D8 still measure the right thing?

A common second-rejection reason is a fixed D4 sitting next to the original D5 actions, which now address a cause nobody believes in anymore.

Step 6: Be realistic about dates

If new data is needed - a capability study, a run-at-rate, a few weeks of production without recurrence - say so, and give a date with an interim status. A resubmission with a credible plan and "effectiveness data to follow on " is often more acceptable than one that claims closure with no data. Check what your customer's process allows; some will accept a staged closure, some will not.

Step 7: Self-review before you press send

Before resubmitting, read the report as the reviewer would, starting from their comments:

1. Is every comment answered, with a location in the report?
2. Is every claim in D4 backed by something attached or referenced?
3. Does each corrective action point to a specific verified cause?
4. Is containment shown as complete, with quantities and dates?
5. Is there effectiveness evidence, or a dated plan to provide it?
6. Is the revision history visible?

If you work alone, this is the hard part: the same person who wrote the report is now judging it, under time pressure, slightly annoyed. That is exactly when gaps slip through.
