Health Wearables Are Getting Smarter: How Wearable Health Technology Is Improving

KEY TAKEAWAYS

• Health wearables have shifted from fitness accessories to passive medical monitors: watches now flag hypertension and sleep apnea, and rings catch early signs of illness before symptoms appear.
• The AI layer is now as important as the sensors, turning continuous biometric streams into readiness scores, sleep coaching and early-warning alerts.
• Global wearable shipments reached 611.5 million units in 2025, up 9.1 percent, and the category keeps moving from simple tracking toward clinical-grade insight.

Health wearables have moved far beyond step counting. In 2026, smartwatches and rings passively track blood pressure trends, sleep apnea, stress and recovery, while AI turns that stream into guidance you can actually use.

Ten years ago, a wearable told you how many steps you took and little else. Today the numbers show a mature platform: IDC counted 611.5 million wearable devices shipped worldwide in 2025, up 9.1 percent from the year before, and 46 percent of US adults now own a wearable, up from 13 percent in 2015. Activity tracking (35 percent), sleep (26 percent) and heart rate (21 percent) are the top uses owners report, and the devices keep earning their place on the wrist by measuring more, more accurately and with less effort from the wearer.

Sensors keep getting smaller and sharper

The core reason health wearables keep improving is hardware. The optical heart-rate sensors in a modern watch sample blood flow through photoplethysmography, measuring the tiny changes in light absorption caused by each pulse. Stacked next to them now sit electrical heart sensors for ECG readings, red and infrared LEDs for blood oxygen (SpO2), skin temperature sensors, accelerometers, gyroscopes and, on some models, bioelectrical impedance sensors that send a small current through the wrist to estimate body composition. One study of more than 1,000 people comparing heart-rate accuracy across major wearables found the Apple Watch measured with the highest accuracy, a sign of how much the gap between consumer devices and clinical tools has narrowed.

That density of sensors is only possible because everything shrank. The Oura Ring 4 packs green, red and infrared PPG sensors, an SpO2 sensor, a body temperature sensor and an accelerometer into a titanium band a few grams in weight, with recessed sensors that sit flush against the finger. Samsung's BioActive sensor combines heart rate, ECG, blood oxygen and body composition analysis in a single module. This miniaturization is not limited to wrist devices; rings, earbuds and even glasses now carry health sensors.

Accuracy is improving through software as much as silicon. Samsung published research with Chung-Ang University Gwangmyeong Hospital showing that Galaxy Watch data could predict vasovagal syncope, a common cause of fainting, up to five minutes before an episode with 84.6 percent accuracy. The result matters because it shows wearables moving from recording what already happened to warning about what is about to happen.

What modern health wearables can measure

Blood pressure is one of the biggest prizes in wearable health, and 2026 brought real progress. The Apple Watch Series 11 added hypertension notifications: the watch analyzes data from its optical heart sensor over 30-day periods, looking at how blood vessels respond to each heartbeat, and alerts the wearer if it detects patterns associated with chronic high blood pressure. Apple stresses it is not a direct cuff measurement and not a replacement for clinical diagnosis. Samsung takes a complementary approach on its watches: blood pressure estimates calibrated periodically against a traditional cuff, alongside ECG and irregular heart rhythm notifications.

Sleep has become a second front. Sleep apnea detection, which monitors overnight blood oxygen to flag signs of moderate to severe obstructive sleep apnea, is now built into watches from Apple and Samsung. Apple also added a daily Sleep Score that breaks down sleep quality into something actionable, while Samsung's Bedtime Guidance reads your circadian rhythm and suggests the best time to turn in. Oura's sleep staging has been validated repeatedly against clinical polysomnography in independent testing, and its Readiness Score rolls sleep, activity, heart-rate variability and temperature into a single morning number.

Metabolic tracking is the next frontier, and here honesty matters. As of 2026, no smartwatch or smart ring has been cleared by regulators to measure blood glucose without piercing the skin, and the FDA has warned against devices that claim to do so. What companies are shipping instead are adjacent metrics. Samsung's Galaxy Watch 8 introduced the first Antioxidant Index on a smartwatch: a five-second thumb press measures carotenoid levels in the skin, a proxy for antioxidant status tied to fruit and vegetable intake. The same watch's Vascular Load metric tracks stress on the vascular system during sleep and builds a three-night baseline. True glucose data still comes from dedicated devices, and the trend there is strong: the FDA authorized Abbott's Libre Duo in August 2026, the first wearable to monitor glucose and ketones continuously, and over-the-counter continuous glucose monitors from Dexcom and Abbott have made prescription-free tracking mainstream.

Stress and recovery round out the picture. Daytime stress tracking uses heart-rate variability, skin temperature and motion to flag when pressure is building, and Oura's Symptom Radar watches for deviations from your personal baseline to flag early illness up to a day or two before symptoms appear. The Pixel Watch 4 tracks stress continuously through electrodermal activity, the tiny changes in skin conductivity linked to sweat responses.

How the leading health wearables compare

The comparison hides an important detail: price and subscriptions change the math. The Oura Ring 4 requires a $5.99 per month membership for its full features, while Samsung charges no subscription for its health insights and Google's Pixel Watch 4 gives its readiness score free without a Fitbit Premium plan.

Milestones in wearable health technology

• 2018: The first FDA-cleared consumer wrist ECG arrives, putting clinical heart readings on the wrist.
• 2024: Sleep apnea notifications launch on mainstream watches; the FDA warns that no noninvasive glucose watch or ring is authorized.
• 2025: IDC records 611.5 million wearables shipped, up 9.1 percent; smart-ring shipments jump about 49 percent; Samsung's Galaxy Watch 8 debuts the Antioxidant Index and Vascular Load.
• 2026: Apple Watch gains hypertension notifications and a Sleep Score; the FDA authorizes the first glucose-plus-ketone wearable, Abbott's Libre Duo.

Where wearable health technology goes next

The next leap is software. Apple has put AI at the center of its health strategy, with a redesigned Health app that uses Apple Intelligence to turn years of fitness and biometric data into accessible health and longevity insights, and its newest watches sample heart rate and heart-rate variability at higher frequencies to feed that analysis. Samsung's Energy Score and personal running coach, which grades fitness from 1 to 10 after a 12-minute test and builds a tailored plan, show the same direction: raw numbers matter less than what the device tells you to do about them.

The bridge to actual healthcare is being built too. Samsung is developing a health data sharing hub that would send watch-collected metrics directly to doctors between visits, and with 68 million active Samsung Health users globally, that could make the wrist an early-warning input for primary care. Similar research into prediction, like the vasovagal syncope study, suggests a future where wearables routinely flag risks days before a clinic visit would catch them.

Form factors will keep diversifying. Smart rings already appeal to people who dislike screens and charging interruptions, and smart glasses show how health sensors are migrating to eyewear too. The pattern from the last decade is clear: each new form factor starts as a novelty, then inherits the full sensor stack.

For anyone shopping today, the practical advice is simple. Decide which platform you live on, since the best devices pair tightly with their phone ecosystems. Prioritize the sensors that match your goals. And remember the limits: these are wellness tools with clinical-grade data, not doctors. Use their alerts as prompts to check in with a professional, and the smarter they get, the more valuable that partnership becomes.
