Why AI Decisions Need an Audit Trail
When an AI system influences a consequential decision — a hiring call, a credit outcome, a medical suggestion, an access decision — you must be able to reconstruct what happened: what the system was asked, what it produced, and what a human did about it. This audit trail underpins accountability, handles disputes and complaints, and provides the evidence regulators ask for (European Commission, 2016; European Commission, 2024).
Without a trail you cannot answer the two questions that matter most after an AI-driven decision: "what did the system consider?" and "who was responsible for the outcome?" Both are central to GDPR's right to explanation and the EU AI Act's documentation and logging expectations for high-risk systems.
What to Log
For each AI-influenced decision, capture:
- Timestamp — when the decision occurred.
- User / request ID — who or what triggered it.
- Input data — what the system was given (or a reference to it).
- Model + version — which model/version produced the output.
- Output — what the system returned.
- Confidence — any score or confidence the system attached.
- Human action — what a human reviewed / decided.
- Version of prompt — the prompt or configuration used.
- Outcome — the final decision and its basis.
Non-Negotiables
Record before, not after. Log the input and configuration at the time of the decision, not reconstructed later. Retrospective reconstruction is unreliable.
Version everything. Models and prompts change. Record the exact version that produced each output so you can reproduce it.
Include the human link. Record the human oversight step — who reviewed and approved. For consequential decisions, the human action is the most important field.
Protect the raw data. Store the log immutably (append-only) and control access, so records are trustworthy in a dispute.
Retention
Match retention to your obligations:
GDPR requires an appropriate retention period and limits on storage (European Commission, 2016).
EU AI Act high-risk systems require automatic logging and retaining of logs for the lifecycle of the system (European Commission, 2024).
Balance these with your own data-retention and deletion policies.
A common practice is to retain decision logs for a defined period (often years for regulated decisions) aligned to legal limitation periods and your documented retention schedule.
Practical Implementation
Automate it. Manually recorded decision logs are incomplete. Instrument the system to capture the fields above programmatically from the start.
Make it auditable. Ensure a reviewer can reconstruct the decision chain end-to-end from the log alone.
Report on it. Periodically check that logs are complete and that the fields that matter (human review, version) are actually populated.
Tying It to Governance
The audit trail feeds the wider governance picture. Review logs when you investigate incidents, when you re-score risks, and when you respond to data-subject or regulator requests. The NIST AI RMF's Measure and Manage functions assume you can look back at how a system behaved — the audit trail is what makes that possible (NIST, 2023).
