Whenever a production outage occurs, corporate leaders eagerly proclaim their commitment to a 'blameless post-mortem culture.' Teams gather in conference rooms, draft detailed incident timelines, and assign Jira action items.
Yet six months later, the exact same failure pattern repeats under a slightly different disguise.
This is the Illusion of Learning.
Organizations rarely learn from failure; they merely improve their narrative throughput. They become faster at writing polished incident reports and executing familiar remediation runbooks without ever confronting the structural pathology that caused the incident in the first place.
Why? Because addressing genuine root causes requires changing executive priorities, realigning capital allocation, or acknowledging that a core business assumption is flawed.
True organizational learning requires moving past superficial 'five whys' exercises. It requires examining the incentive structures that made the catastrophic failure inevitable.
Key Takeaways & Architectural FAQs
Q: Why do the same production incidents keep recurring despite blameless post-mortems?
Teams get faster at writing polished incident reports and running familiar remediation playbooks without confronting the structural cause, because fixing the real root cause usually requires changing executive priorities or capital allocation, not just code.
Q: What makes a post-mortem actually prevent repeat failures?
Moving past a surface-level "five whys" exercise to examine the incentive structures that made the failure inevitable in the first place, not just documenting the immediate technical trigger.
