KEY TAKEAWAYS

• AlphaFold's 2024 Nobel Prize validated AI's central role in biology, with over 200 million protein structures freely available to researchers.
• AI-designed drugs are now in human trials, including Insilico Medicine's rentosertib (Phase III) and Isomorphic Labs' oncology candidates.
• Bringing a drug to market costs about $2.6 billion with only an ~11.8% clinical success rate, which is exactly why AI's speed advantage matters.

Artificial intelligence is reshaping how new medicines are found, from protein structures predicted by AlphaFold to AI-designed drugs now entering clinical trials.

The traditional drug discovery pipeline has barely changed in decades: screen thousands of compounds by hand, test them in cells and animals, then hope one survives clinical trials. AI drug discovery is rewriting that sequence at nearly every step. Machine learning models now predict how proteins fold, design novel molecules from scratch, and identify which patients are most likely to respond to a treatment. The shift has drawn Nobel recognition, billions in investment, and the first wave of AI-designed drugs into human testing. Here is how the transformation works, who is leading it, and what still stands in its way.

AlphaFold's Nobel Win: The Foundation of Modern AI Drug Discovery

The symbolic turning point came in October 2024, when the Nobel Prize in Chemistry was awarded to Demis Hassabis and John Jumper of Google DeepMind for AlphaFold, alongside David Baker of the University of Washington for computational protein design. It was arguably the first Nobel recognizing an AI system as the central instrument of discovery.

AlphaFold2, unveiled in 2020, solved one of biology's grandest challenges: predicting a protein's three-dimensional structure from its amino acid sequence. Before it, determining a single structure experimentally could take months or years of work with X-ray crystallography or cryo-electron microscopy. AlphaFold does it in minutes, at accuracy competitive with laboratory methods. DeepMind then made the predictions a public good. The AlphaFold Protein Structure Database, built with EMBL's European Bioinformatics Institute, now gives open access to over 200 million predicted structures, covering nearly every catalogued protein known to science, and has been used by more than 2 million researchers across 190 countries.

Why does protein structure matter for drug design? Most drugs work by binding to a protein target, like a key fitting a lock. Knowing the exact shape of the lock lets chemists design better keys instead of guessing. AlphaFold 3, released in 2024, extended this to interactions between proteins, DNA, RNA, and small molecules. The field has moved from predicting what molecules look like to designing new ones.

Isomorphic Labs and the drug design engine

DeepMind spun out its drug discovery effort as Isomorphic Labs in 2021 under Hassabis. The company's stated ambition is to design medicines end-to-end in silico, with AI replacing much of the trial-and-error of early research. In February 2026, it unveiled the Isomorphic Labs Drug Design Engine (IsoDDE), a system its technical report says doubles AlphaFold 3's performance on protein-ligand structure prediction and predicts binding affinities with higher accuracy than physics-based methods at a fraction of the time and cost.

The engine also tackles problems older methods could not, such as discovering "cryptic pockets" on proteins, hidden binding sites that only open when a drug molecule is present, and modeling how proteins change shape as drugs bind. Isomorphic's pipeline focuses on oncology and immunology, with its first AI-designed candidates approaching or entering first-in-human trials. The company has discovery partnerships with Novartis, Eli Lilly, and Johnson & Johnson, and raised $2.1 billion in May 2026, one of the largest private biotech financings on record.

From AI Drug Discovery Companies to Clinical Trials: The Pipeline in 2026

Structures and models are impressive, but the true test of AI drug discovery is whether AI-designed medicines work in people. By 2026, a genuine pipeline of such candidates exists. The table below summarizes the most advanced programs.

Methodology note: statuses reflect company disclosures and peer-reviewed publications through September 2026. Clinical-stage counts and trial phases change as programs advance or are discontinued; figures are dated to their source disclosures.

Insilico Medicine: the first fully AI-generated drug candidate

The most cited proof-of-concept is Insilico Medicine's rentosertib. Its target, a protein called TNIK implicated in fibrosis, was flagged by the company's PandaOmics target-discovery platform, and the small molecule itself was designed by its generative chemistry system Chemistry42. The company describes it as the first drug candidate where AI led both target discovery and molecular design rather than assisting at one stage.

The Phase IIa trial in idiopathic pulmonary fibrosis, a progressive lung-scarring disease with no cure, enrolled 71 patients and was published in Nature Medicine in June 2025. It met its primary safety endpoint and showed a dose-dependent efficacy signal: patients on the 60 mg once-daily dose gained a mean of 98.4 mL in forced vital capacity (a key lung-function measure) over 12 weeks, while patients on placebo declined by 20.3 mL. The drug has since advanced into Phase III. The company also reported that the program went from target identification to preclinical candidate in about 18 months, a fraction of the traditional multi-year timeline.

A September 2026 analysis in Nature Biotechnology added an unusual twist: reanalysis of banked blood samples from the trial showed the drug shifted six independent proteomic "aging clocks" toward younger biological ages alongside the lung benefit. That is an exploratory biomarker signal, not proof of an anti-aging effect, but it illustrates how AI-discovered targets can sit at the intersection of multiple disease biologies.

Recursion: scale, imaging, and an acquisition

Recursion Pharmaceuticals took a different route. Its Recursion OS platform is built on automated microscopy: robots capture millions of images of cells exposed to genetic and chemical perturbations, and machine-learning models learn the visual signatures of disease biology. In November 2024, Recursion acquired its closest AI-discovery rival, Exscientia, for $688 million in stock, combining what the companies described as more than 10 clinical and preclinical programs.

The combined pipeline shows AI being applied at different stages. REC-4881, a MEK1/2 inhibitor for familial adenomatous polyposis, reported a median 43% reduction in polyp burden at week 13, deepening to 53% at week 25. REC-7735, an inhibitor selective for a mutant form of the cancer-driving protein PI3K-alpha, went from first chemical hit to a cleared Investigational New Drug application in 10 months, with more than 100-fold selectivity for the mutant protein over the healthy version. By mid-2026 the company reported five clinical-stage programs, more than $500 million in realized partnership payments, and partnerships spanning Bayer, Roche/Genentech, Sanofi, and Merck KGaA.

Generative design: the engine under the hood

Underneath these programs sits a shared technical idea: generative AI for molecules. Just as image generators produce pictures from text prompts, chemistry models generate candidate drug structures optimized for multiple properties at once, binding affinity, selectivity for the disease target, metabolic stability, and the practical matter of whether the compound can actually be synthesized in a lab. Human chemists still validate, test, and refine, but the search space explored in weeks would have taken years of manual design.

The Cost, Risk, and Regulation of AI Drug Development

To understand why AI matters here, consider the economics it is trying to fix. The Tufts Center for the Study of Drug Development estimated the average cost of bringing one new drug to market at about $2.6 billion, including the cost of failures and the time value of money, with a clinical approval success rate of just 11.83%, roughly one success for every 8.5 compounds entering clinical testing. Development typically spans 10 to 15 years. Most of that money is spent on candidates that fail, often late, in expensive clinical trials.

AI's economic argument is twofold: compress the early discovery phase from years to months, and kill bad candidates earlier, before the costly trials begin. Better target selection, more precise molecules, and AI-assisted trial design (picking the right patients, predicting who will respond) all attack the failure rate that makes drug development so expensive. The industry has not yet proven this at scale, and 2026's clinical readouts are the first real test. Setbacks are expected; even AI-guided programs will fail in the clinic, and the field's credibility will ultimately rest on approval rates, not press releases.

Regulators are adapting alongside. In May 2023, the FDA published a discussion paper on using artificial intelligence and machine learning in the development of drug and biological products, noting that more than 100 drug and biologic applications submitted in 2021 alone already included AI or ML components, and committing to a risk-based approach. In January 2025, the agency followed with draft guidance on using AI to support regulatory decision-making, built around the concept of "context of use": the more a regulatory decision depends on an AI output, the more evidence the model must carry. Then in January 2026, the FDA and the European Medicines Agency jointly published Guiding Principles of Good AI Practice in Drug Development, ten principles covering data governance, transparency, lifecycle monitoring, and keeping humans accountable for regulatory decisions.

The through-line of the regulatory stance is consistent: agencies are not prohibiting AI in drug development, but they are shifting the burden from permission to proof. Sponsors must document what a model does, show it is fit for that purpose, and monitor it over time.

Where does that leave AI drug discovery in 2026? The infrastructure era is over: protein structures are a solved commodity, and generative design platforms are operational inside the industry's largest partnerships. The clinical era has begun: the first AI-designed candidates are dosing patients, and the next two to three years of trial readouts will determine whether AI genuinely raises the industry's success rate or merely accelerates its early steps. Either way, the $2.6 billion question now has a new kind of answer being tested in humans.
