There's a particular kind of disappointment that hits when you realize the AI was wrong.
Not obviously wrong — not a broken sentence or a missing answer — but wrong in the way that requires you to actually know the subject to catch it. The kind of wrong that erodes trust in a single confident misstatement.
This is the problem of AI output validation. And it's one of the least glamorous, most important problems in the field right now.
Why Validation Is Hard
Let's be honest about why this is difficult. LLMs are designed to generate plausible text. They're optimized to sound right. A model that says "I'm not sure" is penalized in most training regimes compared to one that says "The answer is clearly X." Confidence and correctness are not the same thing, but the model doesn't know that — and neither, frankly, do we, at inference time.
The core tension is this: the more capable a model seems, the more you trust it, and the less likely you are to catch its errors. We build increasingly powerful systems, and each step up in capability makes validation harder, not easier.
This is the trap we need to design around.
The Validation Toolkit
Here's what a mature output validation stack actually looks like in practice.
1. Structural Validation — The Floor, Not the Ceiling
Start here because it's cheap and catches obvious failures. Structural validation checks the shape of the output before you even evaluate its correctness:
- Schema enforcement: Does the JSON match the schema? Are required fields present? This isn't glamorous but it prevents an entire class of downstream crashes.
- Type checking: Output should conform to expected types. If a field should be a date and it's a paragraph, that's a failure even if the paragraph contains a valid date buried inside.
- Length bounds: If the output should be 100 words and it's 2,000, that's information — usually information that the model drifted or got confused by the prompt.
Structural validation doesn't tell you if the answer is right. It tells you if the answer is in the right form. That's table stakes.
2. Self-Consistency Checks
One of the more interesting validation techniques involves asking the model to verify its own work — and actually using the result.
The pattern looks like this: generate the answer, then generate a critique of the answer, then compare. If the model produces a confident answer and then a critique that contradicts it, that's a signal. Not a guarantee of error — sometimes the critique is wrong — but a flag worth investigating.
A stronger variant: generate multiple independent answers to the same prompt and check for agreement. If five runs produce the same conclusion, that's meaningful signal. If they produce five different conclusions, you have a problem that pure confidence metrics won't reveal.
This is expensive — you're essentially paying for multiple inferences — but for high-stakes outputs it's often worth it.
3. Reference Grounding
The most powerful validation is also the most context-dependent: does the output accurately reflect the source material?
For RAG systems, this means measuring the relationship between the retrieved context and the generated answer. Did the model actually use the documents it was given, or did it produce something that sounds plausible but isn't grounded in the retrieved content?
Techniques here include:
- Attribution scoring: Check whether claims in the output can be traced back to specific passages in the source documents.
- NLI-based entailment: Use a Natural Language Inference model to check whether the output is actually supported by the input. This catches a surprisingly large fraction of hallucinations.
- Direct document comparison: For structured claims (names, dates, figures), extract and verify against source material directly.
Reference grounding is where validation moves from "does this look reasonable" to "is this actually correct." It's also where most deployed systems are weakest, because it requires the infrastructure to track provenance end-to-end.
4. Factual Recall vs. Fluency
LLMs are extraordinarily good at producing fluent text that reads well but contains factually incorrect information. This is the hallucination problem in its most dangerous form — not broken output, but confident wrongness.
The solution isn't to make models more cautious. It's to separate the functions. Use the LLM for what it's good at — synthesis, rephrasing, reasoning — and use retrieval or structured lookups for facts that need to be correct. A model that says "Based on the document you uploaded, the Q3 revenue was £2.3 million" is doing something fundamentally different from a model that says "The Q3 revenue was £2.3 million" without citation.
The difference matters. Build systems that expose it.
5. Output Voting and Ensemble Methods
One underused technique in production systems: voting across multiple models or multiple prompts.
If you have access to multiple models of varying capability, run the same query through all of them and compare outputs. Large models tend to be more accurate on hard problems; smaller models are often sufficient for easy ones. Using a small model to validate a large model's output on simple factual queries can be surprisingly effective — the small model won't be fooled by sophisticated wrongness, and if it agrees with the large model, that agreement is meaningful signal.
This is also where dynamic routing pays off. Route easy queries to fast, cheap models; route hard queries to capable models; route all outputs through validation checks calibrated to the query complexity.
The Human Layer
All of this matters, but there's a point where human judgment is irreplaceable, not because AI can't do better, but because correctness is sometimes context-dependent in ways that resist automation.
When the output will influence a medical decision, a legal judgment, a financial transaction — these are domains where the cost of a confident error is high enough to justify human review. Not because humans are infallible, but because humans can ask questions like "is this the right question to be asking?" — a question that most validation systems don't even know how to formulate.
The practical implication: build validation into the system, but also build feedback mechanisms that surface errors to humans who can act on them. The model that learns from its mistakes is more valuable than the model that never makes them, but only if you close the loop.
Where We Are
Output validation is not a solved problem. The techniques above are real, useful, and deployed in serious systems — but they're patchwork. Each one addresses a failure mode; none addresses all of them. The hallucinations continue.
What's changing is that validation is moving from afterthought to architecture. Teams are building systems where validation is not a post-processing step but a first-class concern, where outputs are evaluated alongside generation, where the cost of producing an unvalidated answer is visible and tracked.
This matters because the models are getting more capable faster than validation is getting more sophisticated. We're building more powerful engines and the brakes are still catching up.
The question isn't whether your AI system will produce an incorrect output. It will. The question is whether you'll catch it.
Build for that.
